When seeking consent in different sections, please specify the type of consent required. Use the following formats for each type:

• Session consent: Indicate session consent with a simple yes or no.
  Example: Session consent: [yes/no]
• Treatment consent: Document treatment consent by stating yes or no, and include any relevant advice or instructions regarding procedure consent.
  Example: Treatment consent: [yes/no] (Include any advice or details about procedure consent)
• Manipulation consent: Detail the patient's consent for manipulation, stating yes or no, and include information about VBI testing performed and results obtained prior to manipulation.
  Example: Manipulation consent: [yes/no] (Detail VBI testing performed and results)
